乳晕皮下注射美蓝在乳腺癌前哨淋巴结活检中的临床应用

摘要： 目的探讨使用乳晕皮下注射美蓝在乳腺癌前哨淋巴结的应用价值。方法将1%美蓝2ml分4点乳晕皮下组织内,然后行乳腺癌改良根治术(Achincloss术式)或保乳术,依淋巴管走向找到距乳腺最近的蓝染淋巴结定为SLN,其余为非SLN,分别送病理检查。结果56例患者中,成功检出前哨淋巴结者54例,检出率为96.4%。共检出前哨淋巴结77枚,平均每例1.38枚。SLN预测前哨淋巴结灵敏度为94.4%(52/54),假阴性率为6.1%(2/33),无假阳性者。结论应用乳晕皮下注射美蓝行前哨淋巴结活检可以准确预测早中期乳腺癌患者的淋巴结状态。

Abstract: Objective To inquiry the usefulness of methylene blue subcutaneous injection into areola in sentinel lymph node biopsy for breast cancer.Methods 1%methylene blue 2ml were injected into areola by 4 points before the operation on breast cancer.The lymph node were named as SLN and non-SLN according their locations related to areola.Result The detection rate of sentinel lymph node was 96.4%(54/56).The false negative rate and the false positive rate were 6.1%(2/33) and 0,respectively. Conclusion Subcutaneous injection methylene blue into areola for sentinel lymph node biopsy is helpful for evaluating the lymph node situation of breast cancer.
