摘要: 围术期肺损伤(perioperative lung injury,PLI)是一种常见的临床并发症,尽管近年来在阐明其发病机制方面取得了显著进展,但由于PLI的因素复杂多样,以及其病理过程的异质性,在临床实践中仍缺乏有效的早期诊断和干预方法。当前研究强调了免疫-炎症反应和细胞应激状态在 PLI 的发生和发展中的关键作用。本文系统综述了PLI相关免疫识别、细胞器功能障碍、程序性细胞死亡及修复调控机制的研究进展,旨在为围术期肺保护策略和新干预方法的探索提供理论支持。

Abstract: Perioperative lung injury (PLI) represents a prevalent clinical complication. Although significant advancements have been made in elucidating its pathogenesis in recent years, the development of effective early diagnostic and intervention strategies remains limited in clinical practice. This limitation is primarily attributed to the complexity of the factors inducing PLI and the heterogeneity of its pathological processes. Current research underscores the pivotal roles of immune⁃inflammatory responses and cellular stress states in the initiation and progression of PLI. This article provides a comprehensive review of  the latest advancements in PLI⁃related immune recognition, organelle dysfunction, programmed cell death, and regulatory mechanisms of repair. The aim is to furnish theoretical support for the exploration of perioperative lung protection strategies and the development of novel intervention approaches.
