    Objective To investigate the clinical efficacy of percutaneous vertebroplasty(PVP) combined with posterior decompression in the treatment of multiple jumping spinal metastasis tumor patients. Methods The clinical data of 96 patients with multiple jumping spinal metastasis tumor admitted to our hospital from January 2013 to December 2019 were collected, and intraoperative blood loss, the VAS score, KPS score and ASIA nerve injury grading before and after surgery were observed. Results A total of 96 patients were successfully completed PVP combined with posterior decompression therapy. The VAS score was decreased at 1 month after surgery [(1.0±0.6) points vs (8.5±1.8) points, P<0.001)], and the KPS score was increased at 3 months after surgery [(75.0±18.2) points vs (45.0±10.5) points, P<0.001)]. One month after surgery, the ASIA grades of 87.5% patients were improved, and the proportion of patients with grade D and E were increased from 11.4% and 0% to 40.6% and 29.2%, respectively. Conclusion The treatment of percutaneous vertebroplasty combined with posterior decompression in multiple jumping spinal metastasis tumor patients can significantly relieve pain, improve quality of life, effectively relieve the spinal cord and restore damaged nerve function to various degree.

    Surgical outcomes of en bloc spondylectomy for solitary spinal metastasis

    Objective To evaluate the short-term clinical efficacy and prognosis of patients with solitary spinal metastases who received en bloc spondylectomy(en bloc). Methods The clinical data of patients with solitary spinal metastasis,  who received en bloc resection in our hospital between January 2017 and September 2019,  were analyzed retrospectively,  including the gender,  age,  lesion location,  primary tumor type,  intraoperative bleeding,  and postoperative surgery complications,  as well as the Frankel neurological function score and KPS functional status score before and after surgery. All patients were followed up every 3 months to assess the local progression-free survival (PFS) and overall survival (OS). Results A total of 28 patients were included in this study,  including 17 male and 11 female. The average operation time was (5.78±0.33) h (range: 4.0-7.5 h),  and the average intraoperative blood loss was (1 867.21±613.84) mL (range: 900-3000 mL); 6 cases of postoperative cerebrospinal fluid leakage and 5 cases of local infection occurred after surgery,  of which 3 cases of infection were patients with cerebrospinal fluid leakage,  and all recovered after treatment. The KPS score and Frankel score were significantly improved after the surgery(all P<0.05). The median follow-up time was 15 months (6-33 months),  the 1-year and 2-year local PFS were 87.1% and 68.8%,  respectively,  and the 1-year and 2-year OS were 72.5% and 42.3%,  respectively. Conclusion En bloc resection is a safe and effective way to treat solitary spinal metastases,  with a low incidence of postoperative complications,  good local control rate,  and a significant improvement in the quality of life.

    CT-guided 125 I seed implantation assisted by 3D printing non-coplanar templates in the treatment of spinal metastatic tumors: A clinical application

    Objective To investigate the clinical efficacy of CT-guided 125I seed implantation assisted by three-dimensional printing non-coplanar templates in the treatment of spinal metastatic tumors. Methods A retrospective analysis was performed on patients who underwent CT-guided 125I seed implantation assisted by three-dimensional printing non-coplanar templates in the People′s Hospital of Liaoning Province for the treatment of spinal metastatic tumor from March 2017 to March 2019. Parameters such as the dose received at 90% target area (D90), the volume percentage of 100.00% prescription dose (V100) and the volume percentage of 150% prescription dose (V150) were evaluated for seed implantation quality, and postoperative complications and survival were also observed. Results All the 15 patients were successfully implanted 125I seed implantation under CT guidance assisted by three-dimensional printing non-coplanar templates. The average activity of radioactive particles was (0.68±0.10) mCi, the average D90 (12 195.60±1 654.78) cGy, the average V100(91.69±2.25) %, and the average V150(61.75±11.67) %. Six months after the operation, the disease control rate was 100%, and the objective remission rate was 73.33%. The median local progression-free survival time was 17.00 months(95%CI: 13.42-20.58 months), and the median total survival time 17.00 months(95%CI: 13.42-20.58 months).6 months after operation, NRS score was lower than preoperative [(2.27±1.28) points vs (5.00±1.51) points, P<0.001], while KPS score was higher than preoperative [(86.00±12.42) points and (72.67±11.00) points, P<0.001]. From preoperation to 6 months after operation, ASIA grade was changed from grade C to grade D, from grade C to Grade E, and from grade D to Grade E in each case. Conclusions CT guided 125I seed implantation assisted by three-dimensional printing non-coplanar template in the treatment of spinal metastatic tumor can effectively control local lesions, significantly alleviate pain and partial relief of nerve injury, and complications are rare. It is a safe and effective minimally invasive palliative treatment.

    The effect of percutaneous microwave ablation combined with percutaneous vertebroplasty in the treatment of lumbar metastases

    Objective  To observe the clinical effect of microwave ablation combined with percutaneous vertebroplasty(PVP) in the treatment of lumbar metastases(MWA). Methods A retrospective analysis was performed on the clinical data of 50 patients with metastatic tumors of the lumbar vertebrae admitted to Guangxi Medical University Cancer Hospital from June 2016 to June 2019, including 26 cases of percutaneous vertebroplasty (PVP group), and percutaneous microwave ablation combined 24 cases of skin vertebroplasty (PVP+MWA group). The pain, functional status, bone cement extravasation and postoperative tumor recurrence rate of the two groups of patients were observed. Results Followed up for 6 to 36 months, the VAS score of the PVP group decreased from (7.58±1.06) points before the operation to(3.27±1.40) points 1 month after the operation and(3.08±1.60) points 6 months after the operation; the PVP+MWA group reduced from(7.67±1.05) points before the operation to(3.04±1.20) points 1 month after surgery and(2.96±1.46) points 6 months after surgery. VAS scores of the two groups at 1 month and 6 months after surgery were lower than those before surgery (all P<0.05), but the variance of repeated measurements showed that no statistically significant difference between two groups(F=0.223, P=0.801). One month after surgery, the KPS score of the PVP group was increased by (21.50±11.32) points than that of the preoperative, and the KPS score of the PVP+MWA group was increased by (19.92±13.19) points than that of the preoperative, but there was no statistically significant difference (t=0.457, P=0.650). The bone cement exosmosis rate (12.5% vs 38.5%, P=0.037) and tumor recurrence rate (8.3% vs 30.8%, P=0.048) in PVP+MWA group were both lower than those in PVP group. Conclusions Percutaneous vertebroplasty alone or in combination with microwave ablation can achieve better clinical effects in the treatment of lumbar metastases. The combination of the two has advantages in the reduction of bone cement extravasation and local tumor control.

    Management of perioperative complications in total spinal resection of spinal tumors

    Objective To investigate the perioperative complications and treatment methods of total en bloc spondylectomy(TES) in the treatment of spinal tumors. Methods A total of 32 patients with spinal tumors, who received TES therapy in Sun Yat-Sen University Cancer Center from January 2017 to February 2020, were collected to observe the occurrence and outcome of intraoperative and post-operative complications. Results All the 32 patients completed the operation successfully. The operation time was (298.21±53.31) min (range: 260-380 min), and the intraoperative blood loss was (2 412.45±316.56) mL (range: 1 850-3 200 mL). On the first day after the operation, the Frankel grade of 2 patients increased from preoperative Grade E to Grade C, 3 patients improved from preoperative Grade C to Grade D, and the remaining 27 patients remained unchanged. There was no significant difference in Frankel classification between preoperative and postoperative patients(Z=-1.526, P=0.075). The total incidence of perioperative complications was 43.8% (14/32), all of which turned out to be good after symptomatic treatment and no fatal complications occurred. Conclusion The inci-dence of complications during the perioperative period of total spinal mass resection for spinal tumors is relatively high, which can be alleviated by timely and effective treatment, and the incidence of fatal complications is low.

    Effect of TRPM8 on the invasion and metastasis of hepatocellular carcinoma Huh7 cells and its mechanism

    Objective  To investigate the effect of chronic receptor potential 8 (TRPM8) on the invasion and metastasis of hepatocellular carcinoma(HCC) cells and its related regulation mechanism. Methods A total of 20 pairs of matched HCC tissues and their adjacent tissues in our hospital from March 2017 to December 2019 were collected, and the expression of TRPM8 in HCC tissues was detected by immunohistochemistry and qRT-PCR. HCC Huh7 cells were transfected with shTRPM8(shTRPM8 group), and set up a negative control group (shcontrol group). The ability of cell migration and invasion were detected by scratch healing assay and Transwell chamber assay, respectively; the expression of E-cadherin and vimentin were detected by immunofluo-rescence experiment; the expression of AFP, TRPM8 and AKT/GSK-3β/Snail signal pathway related proteins were detected by Western blot. The constructed Huh7-LV-shTRPM8 HCC cells were injected into BALB/C nude mice through the tail vein as the experimental group(LV-shTRPM8 group), while the nude mouse model established by the injection of Huh7-LV-shNC cells were used as the control group(LV-shNC group). The lung metastasis of nude mice was observed after injection for 6 weeks. Results TRPM8 was highly expressed in HCC tissues and cell lines(P<0.01). Compared with the shcontrol group, the migration and invasion of Huh7 cells after TRPM8 silence were decreased (P<0.01);the expression of E-cadherin increased(P<0.001), while the expression of vimentin decreased (P<0.001); the expression of TRPM8 and Snail proteins were down-regulated(P<0.001), and the phosphorylation degree of AKT and GSK-3β proteins were decreased (P<0.001). TRPM8 silencing decreased the lung metastasis rate, the number of lung metastatic nodules and the expression of AFP and TRPM8 in lung tissues in nude mice (P<0.01). Conclusion TRPM8 can inhibit the invasion and metastasis of HCC Huh7 cells by regulating the AKT/GSK-3β/Snail signal pathway.

    Effect of GATA4 gene methylation on the growth of non-small cell lung cancer and underlying mechanism

    Objective To investigate the methylation status of GATA4 in non-small cell lung cancer(NSCLC), and its effect on NSCLC cells and possible mechanism. Methods Cancer tissues and paracancerous tissues, as well as human lung cancer cell lines A549, HCC827, NCI-H1299 and human normal lung epithelial cells BEAS-2B, of 78 patients with NSCLC who received radical surgery of lung cancer in First Affiliated Hospital of Xinjiang Medical University from July 2017 to June 2018 were collected. The methylation status of GATA4 gene was detected by MSP and qMSP, the expression of GATA4 was detected by RT-PCR and Western blot. The sh-GATA4-1(SH-GATA4 group), sh-NC, GATA4 overexpression vector(pLV-GATA4 group) and empty vector control lentiviral vector(pLV-NC group) were transfected into lung cancer A549 cells, respectively, and a blank control group(Blank group) was set up. The protein expressions of GATA4, p-ERK, ERK, p-p38 and p38 were detected by Western blot; cell viability was detected by CCK-8; cell proliferation was detected by cell clone formation assay; cell apoptosis was detected by flow cytometry. Results The methylation rate of GATA4 gene, expression levels of p-ERK, p-p38 and Bcl-2 proteins in NSCLC tissues were higher than those in paracancerous tissues(P<0.001), while the expression of GATA4 mRNA, GATA4  and Bax proteins were lower(P<0.001). The methylation level of GATA4 gene was positively correlated with the expression of p-ERK, p-p38 and Bcl-2 proteins(P<0.05), and negatively correlated with the expression of GATA4 mRNA, GATA4 and Bax proteins(P<0.05). GATA4 was in a methylated state in NSCLC cells, while human normal lung epithelial cells BEAS-2B was in a demethylated state, and the GATA4 mRNA and protein expressions were lower than those of BEAS-2B cells(P<0.01). Compared with the pLV-NC group, the GATA4 mRNA and protein expression and apoptosis rate were increased in pLV-GATA4 group(P<0.001), while the cell viability, cell clone number, p-ERK/ERK and p-p38/p38 protein expression were decreased(P<0.05); GATA4 mRNA and protein expression and cell apoptosis rate in sh-GATA4 group were lower than those in sh-NC group(P<0.001), while cell viability, cell clone number, p-ERK/ERK and p-p38/p38 protein expression were higher(P<0.05). Conclusion GATA4 gene is hypermethylated in non-small cell lung cancer and in-duces a decrease in GATA4 gene expression. Overexpression of GATA4 may inhibit the proliferation and induce apoptosis of lung cancer cells by regulating MAPK pathway.

    Expression of miR-205-5p in glioma tissues and its effect on biological behaviors of glioma cells

    Objective  To investigate the expression of miR-205-5p in glioma and its regulation on the proliferation, migration, invasion of glioma cells and possible mechanisms. Methods The glioma tissue and adjacent tissue specimens from 58 newly diagnosed glioma patients in The First people′s Hospital of Chenzhou from June 2018 to December 2019 were collected. U251 and T98G cells were transfected with miR-205-5p mimic to construct the overexpressed cell models. The expression level of miR-205-5p in glioma tissues and cell models were detected by RT-qPCR, and the relationship between the expression level and the clinicopathological characteristics of the patients was analyzed. The proliferation, clonal formation, migration and invasion of glioma cells were detected by CCK-8, plate clonal formation assay, scratch test and Transwell assay, respectively. The influence of overexpression of miR-205-5p on Runx2 protein expression level was detected by Western blot. The targeted relationship between miR-205-5p and Runx2 was detected by double luciferase reporter assay. Results miR-205-5p was low expressed in glioma tissues and glioma cells U251 and T98G(all P<0.01), and its expression level was related to the tumor size and pathological grade(both P<0.05). Overexpression of miR-205-5p could significantly inhibit the proliferation, clonal formation, migration and invasion of glioma cells(all P<0.05). Double luciferase reporter assay confirmed that Runx2 was a potential target gene of miR-205-5p. Overexpression of miR-205-5p significantly down-regulated Runx2 protein expression in glioma cells(P<0.001). Conclusions miR-205-5p is low expressed in glioma. Overexpression of miR-205-5p can inhibit the proliferation, clonal formation, migration and invasion of glioma cells, and its mechanism may be related to the targeted regulation of Runx2.

    Expression and clinical significance of β1 and β2 adrenergic receptors in oral squamous cell carcinoma tissues

    Objective To investigate the expression of β1 adrenergic receptors(β1-AR) and β2 adrenergic receptors(β2-AR) in oral squamous cell carcinoma tissues and their clinical significance. Methods A total of 60 cases of squamous cell carcinoma tissues and corresponding para-carcinoma tissues, resected by the oral and maxillofacial surgery of Guangxi Medical University College of Stomatology from July 2017 to July 2018, were collected, and the normal oral mucosal epithelial tissues of 15 non-squamous cell carcinoma patients during the same period were also collected. RT-qPCR and Western blot were used to detect the expression of β1-AR and β2-AR mRNA and protein, and Cox regression was used to analyze their relationship with clinicopathological characteristics and prognosis. Results The relative expression levels of β1-AR, β2-AR proteins and β2-AR mRNA in oral squamous cell carcinoma tissues were higher than those of para-carcinoma tissues(P<0.01) and normal oral mucosal epithelial tissues(P<0.05). The expression levels of β1-AR, β2-AR mRNA and proteins were correlated with clinical stage, and the expression level of β2-AR was also correlated with lymph node metastasis and postoperative recurrence(both P<0.05). The 1-year, 3-year overall survival rate and progression-free survival rate in the β2-AR high expression group were lower than those in the β2-AR low expression group(χ2=3.945, P=0.047; χ2=9.286, P=0.002), but β1-AR expression level was not correlated with patients′overall survival and progression-free survival. After adjusting for potential confounding factors, multivariable Cox regression showed that the low protein expression of β2-AR was a protective factor for overall survival(HR=0.149, 95%CI: 0.028-0.785, P=0.025). Conclusions β1-AR and β2-AR are highly expressed in oral squamous cell carcinoma tissues.  Patients with high expression of β2-AR have a poor prognosis and may be a potential prognostic indicator for oral squamous cell carcinoma.

    Gene regulatory network analysis of m6A reader IGF2BP1 in hepatocellular carcinoma

    Objective To analyze the expression level of m6A reader insulin-like growth factor 2 mRNA-binding protein 1 (IGF2BP1) in hepatocellular carcinoma (HCC) and its impact on the prognosis of HCC patients, and explore the role of IGF2BP1 in hepatocellular carcinoma and its underlying mechanism. Methods The mRNA-seq data of 5 pairs of HCC sample and the data from TCGA-LIHC were used to identify the expression of IGF2BP1 in HCC, and the clinical follow-up data of 343 HCC patients in the TCGA database was used to analyze the effect of IGF2BP1 expression on the overall survival of HCC patients. The TCGA database was used to screen the co-expressed mRNA of IGF2BP1, and the m6Avar website was used to predict the m6A locus of mRNA and its RNA binding protein and other information, and finally constructed the gene regulatory network of IGF2BP1. Results IGF2BP1 gene was up-regulated in HCC (log2FC RNA-seq of HCC=10.684, P<0.001;log2FC TCGA-LIHC =7.032, P<0.001). The median survival time of HCC patients with low IGF2BP1 was 5.84 years, while that of patients with high expression was 4.44 years(P=0.011). The 22 differentially expressed mRNAs had a targeted binding relationship with IGF2BP1, and were positively correlated with its expression level. Among them, 15 highly expressed mRNAs such as HMGA2 could shorten the overall survival of HCC patients. In addition, the potential m6A methylation sites of HMGA2, PEG10, CEP55, RHO, CDC6 and KIF23 genes were located in their 3′-UTR miRNA-binding sites. Conclusions IGF2BP1 is highly expressed in HCC and results in shorter overall survival. IGF2BP1 may up-regulate mRNA expression through m6A methylation and miRNA inhibition, thereby promoting the occurrence of HCC and leading to poor prognosis.

    Assessment of cancer risk and analysis of screening results of cancer screening program in urban Guangxi from 2014 to 2017

    Objective To analyze the results of high risk assessment and clinical screening of cancer screening program in urban Guangxi from 2014 to 2017. Methods The aged 40 to 74 residents from Xixiangtang district and Qingxiu district in Nanning of Guangxi from November 2014 to August 2017 were recruited using the cluster sample method. The “Cancer Prevention Risk Assessment Questionnaire” was used in the survey to assess the five major cancers(lung cancer, upper gastrointestinal cancer, liver cancer, colorectal cancer and breast cancer) high-risk groups, then clinical screening was conducted, and the high risk of each cancer risk rate, screening participation rate and detection rate were analyzed. Results A total of 152 630 participants completed the questionnaires and risk assessments and 72 376(47.62%) people at high-risk. Among, The high risk rates of lung cancer, esophageal cancer, stomach cancer, liver cancer, colorectal cancer and female breast cancer were 25.66%, 17.39%, 26.11%, 28.95%, 22.19% and 32.76%, respectively;men had a higher risk of lung cancer than women, and women had a higher risk of esophageal cancer, stomach cancer, liver cancer and colorectal cancer than men. The clinical screening participation rates of lung cancer, breast cancer, liver cancer, colorectal cancer and upper gastrointestinal cancer were 26.42%, 25.18%, 25.03%, 11.31% and 7.68%, respectively. Clinical screening results showed that the detection rates of suspected lung cancer, suspected liver cancer, breast BI-RADS grade 4-5, colorectal cancer and upper digestive tract cancer were 0.58%, 0.05%, 2.79%, 0.13% and 0.12%, respectively. Conclusions From 2014 to 2017, the participation rate of cancer screening and the detection rate of positive lesions of cancer screening program in urban of Guangxi from 2014 to 2017 are not high. Therefore, it is necessary to improve the awareness of cancer prevention and the participation rate of physical examination and screening in high-risk area, further identify the high-risk cancer population and improve the screening effect.

    Total resection of retroperitoneal liposarcoma for recurrent retroperitoneal liposarcoma: 4 cases report and literature review

    Objective To investigate the clinical value of total retroperitoneal fat resection for recurrent retroperitoneal liposarcoma. Methods The clinical data of 4 patients with recurrent retroperitoneal liposarcoma treated in the Eighth Hospital of Wuhan from January 2019 to December 2019 were retrospectively analyzed and the literature was reviewed. Results All 4 patients completed the operation successfully, the operation lasted for 420 min(range: 240-660  min), and the intraoperative bleeding was 1,350 mL (range:200-3 000 mL); combined resection of 1 organ in 2 cases, combined resection of 2 organs in 1 case, combined resection of 3 organs in 1 case;Clavien-Dindo classification: Ⅲb in 2 cases, including 1 case of pancreatic fistula and 1 case of abdominal bleeding, and Ⅱ level in 2 cases. The average follow-up time was (8.8±1.5) months, and the recurrence-free survival rate was 100%. Conclusion The total resection of retroperitoneal liposarcoma is a safe and feasible procedure for patients with recurrent retroperitoneal liposarcoma.
