外周T细胞淋巴瘤患者血清中Arg-1、iNOS、VEGF的表达及临床意义

Clinical significance of elevated serum levels of Arg-1，iNOS，and VEGF in patients with peripheral T-cell lymphoma

摘要：

 目的 检测外周T细胞淋巴瘤（peripheral T-cell lymphoma，PTCL）患者外周血液精氨酸酶1（arginase-1，Arg-1）、诱生型一氧化氮合成酶（inducible nitric oxide synthase，iNOS）、血管内皮生长因子（vascular endothelial growth factor，VEGF）的表达，并分析其临床意义。方法 应用酶联免疫吸附试验（ELISA）检测25例PTCL患者化疗前、化疗后外周血液Arg-1、iNOS、VEGF的表达，分析其与临床病理特征的关系。以同期15名健康体检者作对照。 结果 PTCL患者血清中Arg-1、iNOS、VEGF的表达水平均明显高于健康对照者（P均＜0.05），且化疗前高于化疗后（P均＜0.05）。结外 侵犯部位≥2个者、有骨髓侵犯者、巨大包块直径＞10 cm者、Ann-Arbor临床分期为Ⅲ～Ⅳ期者、IPI评分为3～5分者血清中Arg-1、iNOS、VEGF的表达水平分别与结外侵犯部位为0～1个者、无骨髓侵犯者、巨大包块直径≤10 cm者、Ann-Arbor临床分期为Ⅰ~Ⅱ期者、IPI评分为0～2分者比较，差异均有统计学意义（P均＜0.05）。但不同年龄及乳酸脱氢酶（lactate dehydrogenase，LDH）水平的PTCL患者血清中3个因子的表达水平差异均无统计学意义（P均＞0.05）。结论 PTCL患者尤其具有高危复发特征的患者血清中Arg-1、iNOS、VEGF含量显著升高，且化疗前高于化疗后，这些因子可能与PTCL发生、发展密切相关。

Abstract:

 Objective To analyze the relationship of serum levels of Arg-1，iNOS，and VEGF with clinical characteristics in patients with peripheral T-cell lymphoma（PTCL）. Methods We compared ELISA-determined serum levels of Arg-1，iNOS，and VEGF in peripheral blood in 25 patients with PTCL before and after chemotherapy. As a control， we measured the same serum levels in 15 healthy individuals. Results PTCL patients showed significantly higher serum levels of Arg-1，iNOS，and VEGF than healthy controls （P＜0.05）. Serum levels in patients decreased significantly after chemotherapy （P＜0.05）. Elevated serum levels of Arg-1，iNOS，and VEGF correlated with extranodal involvement，bone marrow involvement，bulky disease（diameter >10 cm），disease stages Ⅲ-Ⅳ and 3 to 5 IPI score（P＜0.05）. However， elevated serum levels of the three factors did not correlate with age or LDH level（P＞0.05）. Conclusion In PTCL，elevated serum levels of Arg-1，iNOS，and VEGF may correlate with worse clinical characteristics，and chemotherapy can significantly reduce the levels. This three factors may closely correlated with the occurrence and progression of PTCL.
