    21-gene recurrence score in invasive lobular breast cancer

    Objective To analyze the clinical value of 21-gene recurrence score(RS) in invasive lobular breast cancer (ILC).Methods We retrospectively enrolled ILC patients who underwent breast cancer surgery and 21-gene RS testing in the Comprehensive Breast Health Center at Rujin Hospital,Shanghai Jiaotong University School of Medicine between January 2011 and November 2017. Relationships of 21-gene RS with clinicopathological factors and choice of adjuvant chemotherapy were further analyzed in terms of odds ratio (OR) and confidence interval (CI). Results A total of 59 ILC patients who underwent 21-gene RS testing were enrolled,of whom 10 (16.95%) were classified as low risk;30 (50.85%),as intermediate risk;and 19 (32.20%),as high risk. Both univariate and multivariate analysis demonstrated that molecular subtype status was independently associated with high risk based on 21-gene RS (OR=19.90,95%CI:2.42-163.63,P=0.005). Multivariate analysis showed that molecular ILC subtype independently influenced use of adjuvant chemotherapy (OR=14.70,95%CI:1.59-136.23,P=0.018),whereas 21-gene RS did not (P=0.527). Conclusion The 21-gene RS was associated with molecular ILC subtype. Use of adjuvant chemotherapy was related to molecular ILC subtype but not to 21-gene RS,which deserves further clinical study.

    Relationship between nutritional status and postoperative adverse reactions to chemotherapy in breast cancer patients

    Objective To investigate the relationship between nutritional status and postoperative adverse reactions to adjuvant chemotherapy in breast cancer patients. Methods A total of 100 patients receiving adjuvant chemotherapy (AC-T protocol) after breast cancer surgery were selected from January 1,2014 to December 30,2015. Nutritional risk and malnutrition was assessed before chemotherapy based on the nutritional risk screening 2002 instrument and body mass index (BMI). Adverse reactions to chemotherapy and hospitalization time were recorded, and their potential relationships with nutritional status were explored. Results Incidence of undernourishment was 7%,and incidence of nutritional risk was 44%. Of the 100 patients,35% showed grade Ⅲ-Ⅳ gastrointestinal reactions, 63% showed grade Ⅲ-Ⅳ hematotoxicity, and 13% showed neutropenic fever. Incidence of gradeⅢ-Ⅳ  hematological toxicity was significantly higher among patients with nutritional risk than among those without risk(P<0.05). Incidence of neutropenia fever and of adverse events was significantly higher among patients with undernourishment or nutritional risk than among other patients (P<0.05). Average hospital stay was significantly longer among patients with nutritional risk than among those without nutritional risk(P<0.05). Conclusion Breast cancer patients with undernutrition or nutritional risk may be at higher risk of postoperative adverse reactions to adjuvant chemotherapy, leading to longer hospital stay.

    Objective To construct the function module and content of public number for patients with breast cancer throughout multidisciplinary integrated diagnosis and treatment. Methods A public number system was tentatively established based on literature review and current breast cancer management methods at our hospital. The Delphi method was performed twice with 17 experts on breast cancer diagnosis and treatment, and index content was modified based on this expert consultation. Results The positive coefficient after two rounds of consultation was 100%, and the average authoritative coefficient was 0.91. In the end, the public number contained six first-level indicators(Personal information, Trace the treatment,Medical services,Knowledge education, Transitional care, Feedback) and 11 secondary indicators. Conclusions This study provides reliable public number content for patients with breast cancer throughout their diagnosis and treatment. It can provide a theoretical basis for the clinical development of an information system network for patients with breast cancer.

    Objective To investigate the effect of ribosomal protein S6 kinase 4 variants 1 (RSK4m1) on the proliferation and invasion by human breast cancer cell line MDA-MB-231. Methods RSK4m1 was stably overexpressed in human breast cancer cell line MDA-MB-231 using lentivirus encoding RSK4m1. Untreated MDA-MB-231 cells served as a blank control group,MDA-MB-231 cells transfected with empty virus served as a negative control group and MDA-MB-231 cells transfected with the RSK4m1 overexpression cassette served as the experimental group. The expression of RSK4m1 in each group was detected by real-time PCR and Western blot analysis. The proliferation of cells was assayed using the cell counting kit-8 (CCK-8),and cell migration was measured using the wound healing test. Invasion was detected using the Transwell test. Results The mRNA and protein expression of RSK4m1 was significantly up-regulated in the experimental group(P<0.05). The CCK-8 assay showed that the proliferation of the experimental group was significantly lower than that of the negative control group and blank control group at 24,48,72 and 96 h(P<0.05). The wound healing test showed that the closure rate of the cells in the experimental group was significantly lower than that in the blank control group and the negative control group after 24 h of cell culture(P<0.05). The transwell test showed that the invasive ability of the experimental group was significantly lower than that in the blank control group and the negative control group (P<0.05). Conclusions Overexpression of RSK4 variant 1 can effectively suppress cell proliferation,migration and invasion by MDA-MB-231 cells,which provides an experimental basis for researching RSK4 variant 1 as a new target in the treatment of human breast cancer.

    miR-140-5p attenuates 5-FU-resistance of colon cancer cells by targeting Nrf2

    Objective To evaluate the effect of miR-140-5p on the 5-FU sensitivity of SW480 cells and 5-FU-resistant SW480 cells,and to explore whether the effects of miR-140-5p involve Nrf2. Methods We constructed the 5-FU-resistant cell line SW480/5-FU from SW480 colon cancer cells. Expression of miR-140-5p was detected using qRT-PCR,and sensitivity of the two cell lines to 5-FU was measured using the CCK-8 and cell colony formation assays. Possible relationships between miR-140-5p and Nrf2 expression were explored using qRT-PCR,Western blotting and a dual-luciferase reporter gene assay. The involvement of Nrf2 in regulation of 5-FU-resistance by miR-140-5p was tested in CCK-8 and cell colony formation assays. Results SW480/5-FU cells showed strong 5-FU resistance,which qRT-PCR linked to much lower miR-140-5p expression. CCK-8 and cell colony formation assays showed that miR-140-5p expression correlated with significantly higher 5-FU sensitivity in SW480/5-FU cells. The combination of qRT-PCR,Western blotting and dual-luciferase reporter gene assays suggested that miR-140-5p binds directly to Nrf2 and inhibits it. Overexpression of Nrf2 in SW480/5-FU cells rescued 5-FU sensitivity,based on CCK-8 and cell colony formation assays. Conclusion It appears that miR-140-5p expression is associated with higher 5-FU sensitivity in SW480 cells,and that miR-140-5p sensitizes cells by binding and inhibiting Nrf2.

    Prediction and identification of HLA-A2-restricted CTL epitopes from ovarian cancer-associated antigen TM4SF1

     Objective To predict and identify HLA-A2-restricted CTL epitopes from ovarian cancer-associated antigen TM4SF1. Methods Four prediction programs(BIMAS,SYFPEITHI,IEDB,PROPRED I)were used to predict HLA-A2-restricted CTL epitopes of TM4SF1. Immunoreactivity of these predicted epitopes was measured using ELISOPT directly and in culture. Results Ten candidate CTL epitopes (P1-P10) were screened,and four (P1,P2,P8,P10) were analyzed further for immunoreactivity. SFC was significantly higher in culture ELISPOT than direct ELISPOT for the positive control peptide (322±8 vs 169±22,P<0.05),epitope P1 (114±10 vs 39±7,P<0.05),and epitope P10 (156±31 vs 52±8,P<0.05). Similarly,average spot size was significantly greater in culture ELISPOT than direct ELISPOT for the positive control peptide (21.91±2.45 vs 13.80±1.76,P<0.05),P1 (12.90±0.88 vs 8.31±1.40,P<0.05),and P10 (17.50±3.85 vs 11.96±0.61,P<0.05). These results show that culture ELISPOT is more sensitive than direct ELISPOT,and that the SFC T value is higher for epitope P10 than P1. Conclusions Combining four prediction programs is an effective strategy to identify better candidate epitopes and avoid limitations of a single prediction program. Culture ELISPOT is more sensitive than the direct assay for verifying the immunoreactivity of predicted epitopes. The immunoreactivity of P10 is strongest.

    Objective To investigate the relationship between single-nucleotide polymorphism in the grown hormone receptor (GHR) gene and risk of hepatocellular carcinoma (HCC) in pedigrees with liver cancer in Fusui County,Guangxi. Methods A total of 25 families showing high incidence of liver cancer (106 cases) served as a case group,while 17 healthy families (80 people) served as controls. The GHR gene was genotyped at the locus rs6451620,and allele frequency distribution was determined using time-of-flight mass spectrometry. Logistic regression was used to explore possible relationships between polymorphism at this locus and genetic susceptibility to hepatocellular carcinoma. Results Two alleles A and G and three genotypes AA,GA and GG were identified at the locus rs6451620. Genotype frequency was in accordance with genetic balance and was representative of the Zhuang population in Guangxi. Allele frequency at rs6451620 was 36% for A and 64% for G among HCC patients;the corresponding frequencies among controls were 29.4% and 70.6%. Risk of HCC was 0.81 times higher among individuals with the A allele at rs6451620 than among individuals with the G allele (95%CI:0.27-2.41,P=0.699). Relative to the normal control group,risk of liver cancer was1.49 times higher among cases with an A allele than among those with a G allele (95% CI:0.73-3.04,P=0.274). Conclusions Polymorphism at locus rs6451620 in the GHR gene is no associated with risk of HCC in Fusui county,Guangxi.

    Expression of nuclear factor erythroid-2-related factor 2 in peripheral T-cell lymphoma and its clinical significance

    Objective To investigate the expression and its clinical significance of nuclear factor erythroid-2-related factor 2(Nrf2) in peripheral T-cell lymphoma(PTCL). Methods  Expression of Nrf2 was detected by immunohistochemistry in 24 patients with PTCL and 17 patients with reactive hyperplasia of lymph nodes,and associations between expression level and clinico-pathological features were analyzed. All data were analyzed using SPSS 23.0 software. Results The positive rate of Nrf2 expression was significantly higher in PTCL (41.67%) than in reactive hyperplasia of lymph node tissues (5.88%,P<0.05). Expression of Nrf2 was related to Ann-Arbor clinical stage and B symptom(P<0.05),while it was unrelated to patient age,gender or LDH(P>0.05). Conclusions The increased expression of Nrf2 in PTCL suggests that Nrf2 may play an important role in the development of PTCL.

    Expression of APOF and IGFALS in primary hepatocellular carcinoma and its clinical significance

    Objective To investigate the expression and clinical significance of apolipoprotein F(APOF) and human insulin-like growth factor acid labile subunit (IGFALS) in primary hepatocellular carcinoma. Methods Levels of APOF and IGFALS mRNA were examined in 88 cases of human liver cancer and adjacent tissues and 6 cases of normal liver tissue using quantitative real-time PCR. Relationships between mRNA levels and clinico-pathological factors were analyzed. Results Levels of APOF mRNA were significantly lower in hepatocellular carcinoma tissues (0.156 ± 0.019) than in paracancerous tissue (0.971±0.018) or normal liver tissue (1.140±0.092). Similarly,levels of IGFALS mRNA were significantly lower in hepatocellular carcinoma(0.111±0.016) than in adjacent tissues(1.090±0.054) or normal liver tissue (1.101±0.211,all P<0.05). Expression of APOF was associated with portal vein invasion(P=0.004),while expression of IGFALS was associated with degree of differentiation and portal vein invasion (P<0.05). Expression of APOF and IGFALS was lower among patients showing portal vein invasion than among those without portal vein invasion (P<0.05). Expression of IGFALS was lower in the low-differentiation group than in the high-differentiation group(P<0.05). Conclusions APOF and IGFALS are down-regulated in hepatocellular carcinoma tissues. Expression of the two genes positively correlates with each other,and their expression correlates with the extent of portal vein invasion and differentiation. These genes may be involved in the occurrence and development of hepatocellular carcinoma.

    Objective To identify genetic risk factors for prostate cancer in the Zhuang minority by performing whole exome sequencing on prostate cancer tissues and adjacent tissues. Methods Cancer tissues and adjacent tissues were collected from 26 Zhuang individuals with prostate cancer. All exons were sequenced using the Illuminate platform. Mutated genes were screened,TOP 60 candidate genes were selected,and these candidates were examined for association with human disease based on analysis with the DAVID online function pathway enrichment database and an integrated genetic and clinical database for coding variants in the human genome. Results Co-mutation analysis showed that 23.1% of patients possessed a mutation in low-density lipoprotein receptor-related protein 1(LRP1). An integrated database linking human genetic variation with clinical information showed that the LRP1 mutation at chr12:57556206 may be associated with prostate cancer. This analysis also identified five chr12 sites independent of 57556206 that may be related to prostate cancer: 57603908,57548363,57567636,57554683,and 57549979. Conclusion Mutations in the LRP1 gene may be related to prostate cancer in the Zhuang population.

    Factors influencing image T residue at the end of intensity-modulated radiotherapy for nasopharyngeal carcinoma

    Objective To analyze factors influencing imaging T residual in nasopharyngeal carcinoma (NPC) patients. Methods From June 2008 to June 2016,691 NPC patients were collected. The chi-squared test was used to compare residual rates among groups,and factors affecting imaging T residual were identified using logistic regression. Results Residual rate was significantly higher among patients older than 50 yr than among those younger,and it was significantly higher among men than women (P<0.05). The rate also differed significantly among clinical stages and T stage. Rate differed significantly among patients with different tumor staging parameters (oropharyngeal,hypopharyngeal,nasal cavity,anterior vertebral muscle,carotid sheath area,medial pterygoid muscle,lateral pterygoid muscle,skull base,paranasal sinus,infratemporal fossa,intracranial,cavernous sinus,cranial nerve). Rate was significantly higher among patients with metastatic tumors than among those with non-metastatic tumors (P<0.05). Multifactorial analysis identified the following factors influencing nasopharyngeal imaging T (P<0.05): gender,age,tumor staging parameters (oropharynx,skull base,cavernous sinus and cranial nerve). Conclusion Risk factors of nasopharyngeal imaging T residual were male gender,older age and metastasis to oropharynx,base of the skull,cavernous sinus and cranial nerve.

    Correlation of serum adiponectin level and type I endometrial cancer

    Objective To investigate the relationship between serum adiponectin and type I endometrial cancer. Methods Medical records were retrospectively reviewed for 34 patients with newly diagnosed endometrial cancer and 24 women with normal endometrium at the Affilated Tumor Hospital of Guangxi Medical University. The two groups were compared in terms of serum adiponectin,fasting serum insulin(FINS),and fasting blood glucose(FBG). Logistic regression was used to explore relationships between serum adiponectin levels and type I endometrial cancer. Results Serum adiponectin levels in patients were independent of clinico-pathological features such as surgical stage,pathology grade,myometrial invasion,cervical involvement,and lymph node metastasis(P>0.05). The control group showed a high adiponectin level. Univariate logistic regression showed that relative to individuals with high adiponectin levels,those with low adiponectin levels were at significantly higher risk of endometrial cancer(OR=8.125,95%CI:1.874-35.233,P=0.005), as were those with intermediate adiponectin levels(OR=4.024,95%CI:1.060-15.278,P=0.041). Similar results were obtained after adjusting for fasting insulin levels or HOMA-IR index. Conclusion Risk of endometrial cancer seems to vary inversely with serum adiponectin level. Low serum adiponectin levels may promote the development of endometrial cancer.

    Meta-analysis of efficacy and safety of bevacizumab-based maintenance therapy in patients with advanced colorectal cancer

    Objective  To evaluate systematically the efficacy and safety of different bevacizumab-based maintenance strategies in patients with advanced colorectal cancer. Methods The databases PubMed,EMbase,Web of Science,and Cochrane Library were searched for randomized controlled trials published before July 31,2017 involving bevacizumab-based maintenance treatment of advanced colorectal cancer. Data from included studies were extracted,studies were evaluated for quality using the Jadad score,and data were analyzed statistically using RevMan 5.3. Results In the end,eight trials involving 2,644 patients were included in the analysis. Relative to no treatment (NT),bevacizumab-based maintenance therapy (MT;bevacizumab without or with chemotherapy) was associated with significant improvement in progression-free survival(HR=0.65,95%CI:0.53-0.78,P<0.001) and overall survival(HR=0.83,95%CI:0.71-0.98,P=0.020). At the same time,MT was associated with more frequent grade 3/4 sensory neuropathy(P=0.001) and hypertension (P=0.008). MT and continuous therapy(CT) were associated with similar PFS (HR=1.05,95%CI:0.56-1.71,P=0.830) and OS (HR=1.11,95%CI:0.92-1.35,P=0.270). CT was associated with higher frequency of grade 3/4 sensory neuropathy (P<0.001),fatigue (P=0.020) and hand-foot skin syndrome (P=0.040). Compared with the group receiving bevacizumab alone,the group receiving bevacizumab plus erlotinib showed better PFS (HR=0.81,95%CI:0.67-0.96,P=0.020) and OS (HR=0.81,95%CI:0.67-0.99,P=0.040),albeit with higher incidence of grade 3-4 skin rash(P<0.001). Conclusions Bevacizumab alone or combined with chemotherapy or erlotinib can significantly improve PFS and OS in patients with advanced colorectal cancer,and most patients tolerate the regimen. Bevacizumab-based maintenance therapy can be recommended as an optimal regimen during the maintenance stage.
