    The expression of HSP 70 in rat lymphocytes and its relationship with liver function and blood cortisol after surgical trauma

    Objective To explore the protective effect of HSP 70 in surgical trauma and the relationship between expression of HSP 70 and other stress response.Methods Four-hundred SD rats with different extents of hepatectomy were randomly divided into control group,mild trauma group,moderate trauma group and severe trauma group,100 rats in each group.Taking blood samples separately in each group at 12h before operation as well as 2h,12h,24h and 48h after operation.HSP 70 in lymphocytes,blood ALT,TB and cortisol were detected,and the relationships among them were analyzed.ResultsThe expression of HSP 70 was related to liver function and blood cortisol.ConclusionsHSP 70 plays an important role in inhibiting the deterioration of liver function,and there might be synergy between HSP 70 and Glucocorticoid.

    Effects of neoadjuvant chemotherapy for locally advanced breast cancer

    Objective To investigate the effects of neoadjuvant chemotherapy(NAC)for locally advanced breast cancer(LABC).MethodsA total of 522 patients with LABC collected from January 1996 to December 2006 were enrolled and assigned randomly into NAC(T)group and control(C)group.Breast-conserving surgery(BCS)and total mastectomy(MT)were performed selectively for T group,after 4-6 cycles of NAC treatment that consist of FEC(5-Fluorouracil,Epirubicin,Cyclophosphamide)or TEC(Docetaxel,Epirubicin,Cyclophosphamide).MT was used only for C group.The rate of BCS and prognosis of LABC after NAC were analyzed.Results The response rate in T group was 89.19%(231/259),82.20%(217/264)of the patients became eligible for BCS after NAC among which 85 patients underwent BCS.After following-up for 36~166(median 67.4)months,there showed no significant difference between the patients in T and C group on the rates of recurrence,metastasis,overall survival and disease-free survival.The rates of overall survival and disease-free survival for complete clinical responders were 92.64%(63/68)and 88.23%(60/68)respectively,for incomplete clinical responders were 81.15%(155/191)and 73.82%(141/191)respectively,and differences were significant(P=0.026;P=0.017).Similarly,the rates of overall survival and disease-free survival in the pathological complete response(PCR)group were 97.14%(34/35)and 94.19%(33/35)respectively,while in the group that did not achieve PCR were 81.25%(182/224)and 75%(168/224)respectively,the differences between the two groups were significant(P=0.019;P=0.015).ConclusionsNeoadjuvant chemotherapy for patients with LABC can reduce the tumor size,improve the rate of breast conservation.To precisely assess the stage before treatment is crucial for judging the effects of NAC on prognosis of LABC.

    Effect of thalidomide on serum VEGF level of patients with advanced non-small cell lung cancer and its clinical significance

    Objective To investigate the efficacy of thalidomide combined with cisplatin-based chemotherapy,and the value of serum levels of vascular endothelial growth factor(VEGF)of the patients with advanced non-small cell lung cancer(NSCLC).Methods Sixty-six patients with histological or cytological confirmed diagnosis of stage ⅢB or Ⅳ NSCLC were enrolled in this study.All of the patients received no prior chemotherapy or radiotherapy or surgery.Thirty-five of the patients received thalidomide combined with platin-based chemotherapy(combined group),and the rest received platin only(control group).Serum VEGF levels were assessed by ELISA.ResultsThe response rates were 45.7% and 38.7% in combined group and control group respectively,the difference between the two groups was not statistically significant(P>0.05).Higher clinical benefit rate was found in combined group(77.1% vs 58.1%;P=0.048).The median time to progression(mTTP)in the combined group was significantly longer than that in the control group(114 days vs 89 days;P=0.033).The mean VEGF serum level in both groups decreased significantly after two courses of treatment(P=0.000).The mean VEGF serum level of the patients treated with thalidomide was not significantly lower than that without thalidomide(P=0.513).ConclusionThe treatment of thalidomide combined with platin-based chemotherapy can improve the clinical benefit and mTTP of NSCLC patient.However,the evidence that thalidomide can reduce the levels of serum VEGF is not obvious.

    Preliminary study on biomarkers for detecting malignant pleural effusion by flight mass spectrometry(SELDI-TOF-MS)

    Objective To search specific protein biomarker for detecting malignant pleural effusion by SELDI-TOF-MS.Methods Thirty cases of pleural effusion and thirty controls were matched by gender and age.WCX2 protein chip was used to capture proteins,followed by PBSⅡ analysis.ResultsComparing to normal control,five protein biomarkers were identified which included three highly expressed proteins with the molecular weights as 5335,7577 and 11670Da,respectively.ConclusionsSome protein biomarkers with high sensitivity and specificity can be screened out by SELDI-TOF-MS technique.SELDI-TOF-MS provides a new method for early detection of malignant pleural effusion.

    Breast conservative surgery versus modified radical mastectomy for elderly patients with breast cancer:a systematic review

    Objective To review the efficacy of breast conservative surgery(BCS),compared with modified radical mastectomy(MRM)for elderly patients with breast cancer.MethodsLiteratures related to clinical trials of comparing BCS and MRM on elderly patients with breast cancer were searched from Cochrane Library(Issue 1,2009),MEDLINE(PubMed 1965~2009.5.2),Embase(1974~2009.5.2),CBM-disc(1978~2009.5.2)and CNKI(1979~2009.5.2).The qualities of the literatures were assessed before data collection,and the statistic analysis was conducted by using RevMan 5.0.ResultsTwo studies were enrolled which included a randomized controlled trial(RCT)and a quasi-RCT,and involved totally 343 patients.No significant differences were found between BCS and MRM in terms of survival rate,mortality,local relapse rate and distant metastasis rate at 5-year.While at 10-year,the BCS group was with significantly lower mortality and distant metastasis rate,but two groups were with similar survival rate,local relapse rate and disease free survival rate.ConclusionsWhen elderly patients with solitary breast cancer staged at T1,T2 or T3a and N0,N1a,N1b or M0 are able to tolerate both BCS and MRM,BCS should be a superior option.Since the comparison study on BCS and MRM is limited currently,it is necessary to develop and design more high quality randomized controlled trials.

    Perioperative application of fast-track surgery in patients received colorectal surgery

    Objective To explore the clinical effect of fast-track surgery in perioperative period of the patients with colorectal cancer that received colorectal surgery.Methods164 patients received colorectal surgery were randomly divided into the fast-track surgery(FTS)group and control group,82 in each group.Clinical therapeutic effect and complications of the two groups were compared by each other.Results Comparing to control group,FTS group had significantly shorter time of indwelling stomach catheter and urethral catheter,first feeding via alimentary tract,first exhaust and defecation,and the hospitalization time(all P<0.001).The complication rate was lower in the FTS group than that in control group(P<0.001).The cost of hospitalization in the FTS group was less than that in control group(P<0.001).Conclusion It is clinically feasible to apply fast-track surgery to the patients with colorectal cancer that received colorectal surgery during perioperative period.Fast-track surgery can significantly release pains,reduce complication rate,save medical cost and accelerate recovery after surgery.

    Clinical study of video-assisted thoracoscopic surgery on patients with solitary pulmonary nodule

    Objectives To show the advantage of video-assisted thoracoscopic surgery(VATS)compared with routine exploratory thoracotomy in diagnosis and therapy of indefinite pulmonary solitary nodule(SPN).Methods A retrospective study was performed on 149 cases that were treated in this hospital.All of the patients were without pathologically diagnosed SPN before undergoing thoracoscopy or thoracotomy.Results All patients received lung biopsies and were diagnosed pathologically.There were significant differences between VATS and routine thoracotomy in the average time of analgesis,the duration of chest tube drainage,the total volume of chest tube drainage,the days of hospitalization after operation,the operation minutes of lung lobectomy and the cost of hospitalization.The volume of blood in operation and the operation minutes of wedge resection of lung had no significant difference between the two groups.Conclusions VATS is effective and should be promoted to further apply on the patients with SPN.

    UI trasonography of liver focal nodular hyperplasia

    Objective To investigate the ultrasonographic characteristics of liver focal nodular hyperplasia(FNH),and to evaluate the application of ultrasound in FNH diagnosis.Methods The findings of two-dimensional gray scale ultrasound and color Doppler ultrasound were reviewed retrospectively,which included 16 nodules form 14 patients.All of the nodules were proved pathologically as FNH.ResultsThe detection rate of FNH by two-dimensional gray scale ultrasound was 100%.By conventional grey scaleultrasound,10 nodules located in right lobe of liver,5 nodules located in left lobe of liver and 1 nodule located between right and left lobe of liver,6 nodules were isoechoic or slightly hypoechoic,8 nodules were hypoechoic,and 2nodules were hyperechoic.Color flow with arterial spectrum was detected in 12 nodules,and radiated wheal like color flow was visualized in 4 nodules.The maximal velocity was 58.5cm/s(26.1~121.6cm/s),and the resistent index was 0.51(0.36~0.81).The result of blood tests showed all patients were AFP negative.ConclusionsUl trasonography of FNH is characteristic.Gray scale sonography combined with color Doppler ultrasound and AFP tests can improve the diagnostic accuracy of FNH.

    Objective To evaluate the efficacy of concurrent chemoradiotherapy combined with adjuvant chemotherapy on the patients with nasopharyngeal carcinoma(NPC)at stage III-IVa.Methods 108 patients with NPC at stage III-IVa were randomly divided into two groups.Test group(n=54)was treated with DDP 30mg/m2 and 5-FU 750mg/m2 on day 1-3;radiotherapy started 1-3 days after the end of first phase chemotherapy;the next two phases of chemotherapy were from 5th-week to the end of radiotherapy;adjuvant chemotherapy were performed after 1-4 weeks at the end of radiotherapy and repeated for 2 cycles.Control group(n=54)was treated only with conventional radiotherapy.Results The 5-year overall survival rate and disease-free survival rate were significantly higher in test group than that in control group(63.0% vs 42.6%,61.1% vs 38.9%,P<0.05).The rate of distant metastasis was significantly lower in test group than that in control group(18.5% vs 37.0%,P<0.05).Among the patients at stage N3,the rates of distant metastasis in test group and control group were 57.1%(4/7)and 66.7%(4/6)respectively(P=1.000).The test group experienced significantly more mucositis than that in control group(48.1% vs 27.8%,P<0.05).Conclusions Concurrent chemoradiotherapy combined with adjuvant chemotherapy could improve the 5-year overall survival rate and disease-free survival rate of NPC patients at stage III~IVa,and could reduce the rate of distant metastasis except the patients at stage N3.

    Evaluation the efficacy of four regimens on treating patients with advanced nasopharyngeal carcinoma

    Objective To evaluate the short-term therapeutic effect,toxicity and side effects of Cisplatin+5-Fluorouracil(PF)regimen,Docetaxel+Cisplatin+5-Fluorouracil(DPF)regimen,Gemcitabine+Cisplatin(GP)regimen,and Paclitaxel+CBP(TC)regimen on treating advanced and recurrent nasopharyngeal carcinoma(NPC).Methods Nineteen cases were treated with PF regimen,22 cases with DPF regimen,23 cases with GP regimen and 23 cases with TC regimen.The response rate,the occurrence of leukocytopenia,nausea and vomiting in each group were calculated and statistically analyzed.Result The response rates of PF,DPF,GP,and TC regimen were 42.11%,72.73%,78.26% and 56.5%,respectively.The higher response rates in the regimens of DPF and GP comparing to regimen PF were statistically significant(χ2=3.939,P=0.047;χ2=5.768,P=0.016).The occurrence rate of Ⅲ~Ⅳ grade leukocytopenia was no significantly different among the GP,TC and PF regimens(P>0.05).The difference between regimen DPF and PF was statistically significant(χ2=12.897,P=0.001).The differences of occurrence of nausea and vomiting among the four groups had no statistically significant(P>0.05).Conclusion DPF,GP and TC are effective chemotherapy regimens for advanced and recurrent NPC.

    Prognosis analysis on 125 cases of nasopharyngeal carcinoma treated with later course three-dimensional conformal radiotherapy

    Objective To analyze the therapy effect and influence factor of late course three-dimensional conformal radiotherapy(3DCRT)on untreated patients with nasopharyngeal carcinoma(NPC),and to evaluate the feasibility of this therapy.Methods 125 untreated patients with NPC that received late course 3DCRT from January 1999 to December 2007 were retrospectively analyzed,by calculating the local control rate,distant metastasis-free survival rate,tumor-free survival rate and overall survival rate.The overall survival rate of patients with parapharyngeal space invasion or skull base invasion was also calculated.Results The 3-,5-and 10-year local control rates were 98%,95% and 94%,respectively.The distant metastasis-free survival rates were 96%,94% and 86%,respectively.The tumor-free survival rates were 98%,97% and 89%,respectively.The overall survival rates were 93%,86% and 76%,respectively.Overall survival rates with parapharyngeal space invasion or skull base invasion were 98.0%,94.0%,92.0% and 89.0%,79.0%,50.0% for 3-,5-and 10-year,respectively.Conclusions There is satisfactory effect of late course 3DCRT on the untreated patients with NPC.There is still good survival rate even when the patients were with parapharyngeal space invasion or skull base invasion.3DCRT could improve the treatment benefit ratio without obviously increase the costs comparing to that with conventional radiotherapy.The late course 3DCRT is a good selection for the untreated patients with NPC.

    Effect of hyperthermotherapy combined with TP chemotherapy in treating advanced non-small cell lung cancer

    Objective To evaluate the clinical efficacy,survival rate,life quality and side effects induced by hyperthermotherapy combined with TP chemotherapy in treating the patients with advanced non-small cell lung cancer(NSCLC).Methods Sixty NSCLC patients were randomly divided into two groups.Group one(n=30)was treated with Paclitaxel plus cisplatin(TP)for four cycles,combined with hyperthermotherapy.Group two(n=30)was treated only with regular TP chemotherapy for four cycles.One month after treatment,the efficacy,life quality and side effects were compared between the two groups.Results The response rates in group one and group two were 46.7% and 36.7% respectively,and the difference between the two groups was not statistically significant(P>0.05).Between two groups,the difference of the change of KPS score before and after treatment was statistically significant(P<0.05).The toxic and side effects of the two groups had no significant difference.Conclusion The use of TP chemotherapy combined with hyperthermotherapy can increase efficacy and improve life quality of the patients with advanced NSCLC.
