    Effects of Gabapentin combined with Morphine on neuropathic pain of rabbit

    Objective To investigate the analgesic effect of Gabapentin combined with Morphine on neuropathic pain of rabbit,and the effects on inflammatory cytokines TNF and IL-6.Methods Twenty rabbits with successfully established neuropathic pain were randomly divided into four groups:saline(NS),Gabapentin(G),Morphine(M) and combination of Gabapentin and Morphine(MG).Animals in each group were given the appropriate drug by gastric gavage.Before and after administration,the behavioral changes of the rabbits were observed,while the neurotransmitters 5-HT and NE,inflammatory cytokines TNF-α and IL-6 in serum were measured by ELISA.Result 1.Behavioral change:after administration,the spontaneous pain of rabbits in groups M,G and MG had various degree of improvement,which appeared earliest in MG group,while the G group appeared earlier than M group.The NS group had no significant change.2.Neurotransmitter change:the neurotransmitters in serum reduced in Groups G,M and MG after administration(P<0.05),which appeared earlier in group MG than that in group M and G.Group NS had no significant change.3.Inflammatory cytokines change:after administration,the inflammatory cytokines in serum decreased in groups G,M and MG,which appeared earlier in group MG than that in group M and G.Group NS had no significant change.Conclusions The synergistic effect of Gabapentin and Morphine can effectively relieve neuropathic pain in rabbits,and can reduce the expressions of inflammatory cytokine TNF and IL-6.The analgesic efficacy of Gabapentin combined with Morphine is better than that by using Gabapentin or Morphine alone.

    Objective To investigate the expression of survivin and CD+8 cytotoxic T lymphocytes(CTL) of 5-Aminolevulinic acid(5-ALA) mediated photodynamic therapy(PDT) on mouse nasopharyngeal carcinoma(NPC).Methods Mouse NPC model was established by subcutaneous inoculation with CNE 2 cells into the right dorsal area of BALB/c mouse.32 mice were divided into two groups at random,namely PDT group and control group.The expression of survivin and number of CTL infiltrated in tumor tissues were detected by immunohistochemical staining.Results Expression level of survivin in PDT group was significantly lower than that in control group(P<0.05).Compared to control group,the number of CTL in tumors tissue showed no significant change 24h after treatment(P>0.05).The number of CD+8CTLs in tumor tissues of PDT group was significantly higher than that in control group 72h after treatment(P<0.05).Conclusion Suppressing the expression of survivin and immune efficacy might be one of the mechanisms for treating mouse NPC with 5-ALA mediated PDT.

    Reversion of multidrug resistance by RNA interference targeting HIF-1α gene in ovarian cancer cells

    Objective To explore the feasibility of reversing multidrug resistance by RNA interference targeted on hypoxia inducible factor 1α(HIF-1α) gene in ovarian cancer cells.Methods Small hairpin RNA eukaryotic expression vector targeted on HIF-1α gene was constructed and transfected into COC1/DDP cells with liposome.The mRNA and protein expressions of HIF-1α and MDR-1 were measured by real time PCR and Western blot.The multidrug resistant to anticancer agent Cisplatin was evaluated by MTT assay.Results After transfected with the small hairpin RNA eukaryotic expression vector targeted on HIF-1α gene,the mRNA and protein expressions of HIF-1α and MDR-1 in COC1/DDP cells decreased significantly under hypoxic condition.Furthermore,the sensitivity to Cisplatin significantly increased.Conclusions The small hairpin RNA eukaryotic expression vector targeted on HIF-1α gene can effectively suppress the expressions of HIF-1α and MDR-1 genes in ovarian cancer cells,and then can reverse the multidrug resistance.

    Application of visual inspection with acetic acid staining for cervical cancer screening in rural area

    Objective To investigate the feasibility of visual inspection with acetic acid staining for cervical cancer screening in rural area.Methods Retrospective analysis was performed on the data of cervical cancer screening collected from 2008 to 2009 in Qingyuan city of Guangdong province.Visual inspection with acetic acid(VIA) staining and iodine(VILI) staining were used generally,while the individuals with abnormal result were further performed colposcopy biopsy and pathologic diagnosis.The suspicious individuals screened out in 2008 were reexamined in 2009.The incidences of various grades of lesion tested by visual inspection screening and by the routine screening in our department were compared.Results The rates of compliance population in 2008 and 2009 were 33.38% and 50.71% respectively.The results from pathologic diagnosis on colposcopy biopsy in 2008 and 2009 were as following:normal cases were 52(86.67%) and 51(70.83%) respectively,cases with CIN I were 5(8.83%) and 11(15.28%) respectively,cases with CIN II were 1(1.67%) and 4(5.56%) respectively,cases with CIN III were 2(3.33%) and 5(6.94%) respectively,cases with cervical cancer were 0 and 1 respectively,and the rates of treatment were 33.33% and 30.00% respectively.The rate of reexamination in 2009 was 12.06%,of which pathologically diagnosed as normal were 2 cases(33.33%),CIN I were 3 cases(50.00%),CIN II was 1 case(16.67%) and none with CIN III.There was no statistical significance between the deference of the rates by visual inspection screening in 2008 and by the routine examination in our department,but the difference of the rates by visual inspection screening in 2009 and by the routine examination in our department was statistical significance(P<0.05).Conclusions Visual inspection with acetic acid staining for cervical cancer screening is a cost-effective method in the area where resources is limited and economy is underdeveloped.With propaganda and education of the knowledge on cervical cancer,the compliance of participation and treatment will increase which will make early diagnosis and treatment possible and then the incidence and mortality of cervical cancer will be reduced.

    Prognostic and cosmetic effects of breast-conserving therapy on 176 patients with breast cancer

    Objective To evaluate the prognostic and cosmetic effects of breast-conserving therapy on patients with breast cancer.Methods Data of relapse,metastasis,survival and cosmetic from 176 patients with breast cancer received breast-conserving therapy were analyzed retrospectively.All the patients were in clinical stage 0~IIa,with single lesion in non-areola area.Lumpectomy was adopted with negative margin,followed by 50Gy radiation on whole breast and 10Gy on tumor-bed.Besides,patients carrying invasive breast cancer were given chemotherapy,and those with positive estrogen and/or progesterone receptor were given endocrine therapy.Results All the patients were followed up for a median period of 47.4 months(18~162 months).Ipsilateral breast tumor recurrence(IBTR) was found in 7 cases,distant metastasis was found in 8 cases and 3 of which were dead.The cosmetic results were evaluated according to the criteria of JCRT(Joint Center for Radiation Therapy).169 patients were classed into three grades:good(62.7%),fair(36.1%) and poor(1.2%).138 cases of them were followed up over 3 years.The 3-year overall survival rate was 99.28%,the disease free survival rate was 94.20%.Conclusions Breast-conserving treatment for breast cancer in stage 0~IIa is feasible.The strict and regulated approach is crucial for guaranteeing the success of treatment.

    Efficacy of transcatheter hepatic arterial chemoembolization and intravenous chemotherapy on patients with liver metastases from colorectal cancer

    Objective To analyze the efficacy of transcatheter hepatic arterial chemoembolization(TACE) and intravenous chemotherapy on liver metastases from colorectal cancer,by comparing short-and long-term therapeutic effect on the patients.Methods 62 patients with liver metastases from colorectal cancer were divided into two groups.17 patients who were treated by TACE were defined as TACE group,and the other group included 45 patients who received intravenous chemotherapy.The follow-up Results on therapeutic effect and survival rate from the two groups were compared.Results The short-term efficacy rate in TACE group was 64.7%,while it was 33.3% in intravenous chemotherapy group.The 1-,2-,3-and 5-year survival rates in the group of TACE were 82.4%,32.4%,12.9% and 0,versus 59.6%,19.3%,7.4% and 0 in the group of intravenous chemotherapy.Conclusions TACE and intravenous chemotherapy are effective,safe and alternative treatment for liver metastases from colorectal cancer in which the tumor can not be removed.Short-term therapeutic effect of TACE is better than that of intravenous chemotherapy,but the survival rates of the two treatments are similar.

    Preliminary observation of Herceptin-combined chemotherapy on Her-2 over-expressing metastatic breast cancer

    Objective To observe the effects and toxicity of Herceptin-combine Vinorelbine and other drug treatment on Her-2 over-expressing metastatic breast cancer.Methods Treatment consisted of Vinorelbine(25mg/m2 on day 1 and day 5 respectively),Trastuzumab(4mg/kg on day 0,followed by 2mg/kg weekly),Cisplantin(20mg/m2 on day 1~3) and Fluorouracil(1.0g on day 1~3).Results The overall response rate was 70%,including 6 cases(30%) complete and 8 cases(40%) partial response.Median survival was 15 months.Main toxicity was the degree III~IV of neutropenia(15/20) and secondary fever(6/15).Only one patient showed asymptomatic degree II of cardiac toxicity.Conclusion Herceptin-combined chemotherapy for treating Her-2 over-expressing metastatic breast cancer is safe and reliable,with significant efficacy.

    Risk factors of cardiac arrhythmia following operation on lung cancer

    Objective To investigate the risk factor of cardiac arrhythmia following operation on lung cancer,to search approach for its prevention and treatment.Methods With methods of case-control study,an epidemiological survey of the cardiac arrhythmia following operation on lung cancer in Tumor Hospital of Guangxi Medical University from April 2004 to April 2009 was conducted.Logistic regression was performed.Results Risk factors were identified by multifactor unconditional logistic regression,including age older than 65y,tobacco smoking,poor pulmonary function,pericardial injury,after acid-base imbalance,electrolyte disorders,large scale of clearance of lymph nodes,pathological grading,complications after operation,postoperative analgesia.Conclusion To prevent the occurrence of postoperative arrhythmias,the measures such as active treatment of cardio-pulmonary complication,improvement of heart and lung functions,postoperative care with ECG and preventive treatment are necessary.

    Surgical efficacy of 48 cases of colorectal cancer with liver metastasis

    Objective To probe the curative effect of surgical treatment on colorectal cancer with liver metastasis,the factors that affect postoperative survival rate,and the measures for improving long-term curative effect.Methods Clinical data from 48 cases of colorectal cancer with liver metastasis were analyzed retrospectively,among which 35 cases were treated with radical excision and 13 cases were with non-radical excision.Results 1-,3-and 5-year postoperative survival rates were 91.7%,50.0% and 29.2% respectively.The number of tumor,operation method and postoperative therapy were the important factors for influencing long-term curative effect.Conclusions Early detection and early excision of the metastatic liver cancer is the key to improve healing rate.Comprehensive therapy after operation can improve the long-term curative effect of colorectal cancer with liver metastasis.

    Objective To evaluate the therapeutic effect and clinical significance of three dimensional conformal radiotherapy(3DCRT) on primary liver cancer combined hypersplenism.Methods 63 cases of primary liver cancer combined hypersplenism were randomly distributed to 3DCRT group and control group.The patients in 3DCRT group were treated with DT30Gy/15f,conventional fractionated.The patients in control group were treated by Yixuesheng orally for three weeks.Routine blood test and spleen size check were taken before and after the treatment.Results Among the values of WBC,RBC,HGB and PLT,3DCRT group showed significant differences in RBC and PLT before and after radiotherapy(P<0.05).There was no significant difference in control group before and after drug treatment(P>0.05).Conclusions Hypoleukemia and thrombocytopenia induced by primary liver cancer combined hypersplenism can be treated by 3DCRT.Comparing to surgical and interventional treatments,the adverse reaction of 3DCRT is little and the therapeutic effect is exact.3DCRT is a safe and effective treatment which is worth applying in clinic.

    Paclitaxel-combined chemotherapy in treating advanced breast cancer

    Objective To evaluate the efficacy and toxicity of Paclitaxel(PTX) combined with Adriamycin(ADM) or cisplatin(DDP) in treating advanced breast cancer.Methods Thirty-five patients with advanced breast cancer were treated with PTX 135mg/m2 on d1 plus ADM 40mg/m2 on d1 or DDP 30 mg/m2 on d1~3.One cycle included 21~28 days treatment and each patient was given 2 cycles at least.Results 35 patients were evaluated for clinical response.The response rate in TA group was 52.6%(10/19),in TP group was 43.8%(7/16).The median remission period of TA and TP group was 8.6 and 7.8 months respectively,while the median survival period of TA and TP group was 14.5 and 13.8 months respectively.The differences of efficacy and median response duration between the two groups were not significantly(P>0.05).Main side effects included leukopenia,gastroenteric reaction and alopecia.Conclusions Paclitaxel-combined chemotherapy is effective for treating advanced breast cancer.There is no significant difference between the two programs.Both programs can be used as the first-line treatment for advanced breast cancer.

    The efficacy of different interventions on postoperative levels of immunosuppressive acidic protein and interleukin-6 in patient with anxiety and hepatocellular carcinoma

    Objective To explore the efficacy of different interventions on immune functions of the patients with anxiety and hepatocellular carcinoma(HCC) after operation.Methods Patients with HCC were assessed by self-rating anxiety scale(SAS),then 132 cases were selected into anxiety group(group A) and the matched 132 cases without anxiety were into the control group(group B).Group A and B were further randomly divided into four subgroups,namely psychological plus drug group,psychological group,drug group and non-intervention group.The levels of immunosuppressive acidic protein(IAP) and interleukin-6(IL-6) from each group before and 1-,4-,7-,14-,30-day after operation were compared.Results The preoperative levels of IAP and IL-6 in group A were significantly higher than that in group B,and the differences were statistically significant(P<0.01).Among the subgroups of groups A,the 1-,4-,7-,14-and 30-day postoperative levels of IAP and IL-6 were lower in psychological plus drug group and psychological group than that in drug group and non-intervention group(P<0.05 or P<0.01).Among the subgroups of group B,the differences of postoperative level of IAP and IL-6 were without statistically significant(P>0.05).Conclusions The preoperative anxiety of patient with HCC directly involves in suppression of immune function,and it has synergistic effect with surgery attack.Both of psychological intervention alone and psychological plus drug intervention can regulate patient's immune function and promote postoperative recovery.
